Affirmative Action Assurance Application

The City of Dayton requires an Affirmative Action Assurance (AAA) form to be approved by the Human Relations Council for all entities that may be awarded contracts involving any expenditure or cumulative expenditures in a calendar year requiring City Commission approval.

The AAA form must be completed electronically via the City’s online vendor portal, CityBOTS (www.citybots.com). Older paper/PDF versions of the AAA form are no longer being accepted.


To obtain your CityBOTS login credentials: Complete a Vendor Application, W9, and EEO Form

(be sure to provide a valid email address) and submit it to the City’s Purchasing Department.

    Once your Vendor Application is processed (within 5 business days), you will automatically receive an email from “support@citybots.com” with your CityBOTS login credentials.

    If you are an existing Vendor and have not received your CityBOTS login credentials, please email the Business & Technical Assistance team at bta@daytonohio.gov with your company’s EIN, along with the name and email address of the person who will be completing the online AAA form.


    To complete your AAA form online: Log onto CityBOTS and click on the icon marked “AAA” to access the online application form.

    In order to complete the AAA form, you will need:

    • A PDF copy of your company’s existing Affirmative Action / Equal Employment Opportunity (EEO) policy. You will be asked to upload a copy of this policy if you have not already done so. (If your company does not currently have an Affirmative Action / EEO policy, you will be given the option to, within 90 days, adopt our sample EEO policy and distribute it to your workforce.)
    • Demographic data about your company’s current workforce. This is similar to the workforce demographic data included in the U.S. Department of Labor’s EEO-1 Form.

    Once you submit your AAA form for approval, we will process it within 5 business days. You will then receive an email explaining the status of your AAA certification.
